    The Home First Support Service provides personal care and reablement support to older people on a short-term basis as a means of either avoiding hospital admission or supporting discharge from hospital.

    Job Role

    There will be shift work, including working up to 6pm on weekday’s and weekend work

    The post requires as a minimum a Diploma level 2 in Health & Social Care or equivalent.

    ICT skill are essential although additional training will be provided.

    A full valid driving license and use of a car is required for this posts as there will be extensive travel across Cardiff and the Vale.

    28 hours

    (Funded until 31st March 2018)

    This posts will be subject to a satisfactory Enhanced Disclosure check.

    Interview dates – 8th & 9th August 2017

    The Service is registered as a Domiciliary Care Agency with CSSIW.
