Our team of staff and volunteers deliver a range of services to people affected by dementia across Gwent. They are supported by an Operations Manager and Services Managers.

    Job Role

    We are looking for an exceptional individual to join our Dementia Support team you will be based in Newport. The service you will provide will involve giving support, information and guidance to a person with dementia or their carers. The successful candidate will provide support to people with dementia and their carers in the Newport area, helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. You will assist people with dementia and their carers to identify their needs and to access services. This could be in the client's own home, at service offices or at other locations, face to face, by phone, letter or email. The service may include signposting and providing referrals to other services. You will also manage referrals from both internal and external professionals, assisting clients in assessing their information and support.


    The successful applicant will need to demonstrate an ability to build effective working relationships and be able to work as part of both internal and external teams.


    The successful applicant will possess excellent communication skills and an understanding of person centred approach to the needs of people with memory difficulties/dementia and have experience of face-to-face service delivery, a broad understanding of statutory and voluntary services, ensuring independence, choice and control for all those who access our service. They will be committed to supporting people with dementia to live better lives.


    You will possess an NVQ Level 3 or have equivalent experience in a Health and Social care setting. You will have good organisational and time management skills and experience of record keeping and report writing.


    You must be willing and able to travel independently across Newport, and occasionally across Gwent.
